	For more than 25 years, Health Alliance has offered quality health coverage. We are the largest managed care organization based in downstate Illinois. Health Alliance covers more than 250,000 lives in Illinois and Iowa enrolled through more than 2,000 employer groups.
	Incorporated in 1996, Stratum Med is a physician owned and governed services company with Shareholders located in Indiana, Illinois, Iowa, Michigan, Minnesota, Ohio and Wisconsin. Stratum was formed by nine highly successful Midwestern medical groups. Currently, there are approximately 2,400 physicians associated with Stratum.
