Confidentiality
Confidentiality can be a major concern for individuals who seek help with an addiction. At the Center, your information is kept strictly confidential. The Carle Addiction Recovery Center's medical records are kept separate from other Carle Clinic or other hospital records.
Maintaining the confidentiality of your protected health information is important. If you are seeking medical records for you or a family member, Carle Clinic Association and Carle Foundation Hospital require a written request to authorize a record release.
Download the Carle Addiction Recovery Authority to Release Information form.
The authorization will expire 90 days from the date signed unless otherwise specified. If you have any questions about completing forms or obtaining copies of your medical records, call Health Information Management at (217) 383-3381.
Finances
Fortunately, most insurance companies recognize addiction as an illness and provide coverage for treatment expenses. Our Business Office Representative is available to help you determine the extent of your insurance coverage.
