Patient Referral

Referring a patient? The Physician Access Line at (800) 451-4330 remains your connection for all your referral needs.

• Press 1 to schedule an outpatient appointment with a Carle specialist. Staff is available weekdays between 7 a.m. and 5 p.m.

• Press 2 for hospital admits or transfers.

After you provide the necessary information, a Carle scheduling center specialist will:

  • Coordinate the appointment or consultation requested
  • Confirm appointment details with you and your patient
  • Provide any additional information necessary to prepare the patient for the appointment
  • Arrange for a telephone consultation with a Carle physician if needed
