Patient Referral
At Carle Clinic, the patient referral process simple and easy for our community physician partners. Call our Physician Access Line (PAL) at (800) 451-4330 to refer a patient.
After the necessary information is provided, a Carle Clinic scheduling center specialist will:
- Coordinate the appointment or consultation requested
- Confirm appointment details with you and your patient
- Provide any additional information necessary to prepare the patient for the appointment

The scheduling center specialist can also arrange for a telephone consultation with a Carle Clinic physician. Carle Clinic Scheduling Center is available Monday – Friday, 7:30 a.m. – 5 p.m. by calling (217) 326-1894. After hours, please call (800) 451-4330.
